To be held at Barking Learning Centre, 2 Town Square, Barking IG11 7NB
In 2014, Barking & Dagenham Health & Wellbeing Board signed up to a challenging target to reduce unplanned admissions to hospital, as a key aim of our better Care Fund programme. To date, we have struggled to continue the strong track record of reduction over past years. This is likely to cost the partnership £700,000 in performance penalties this year. The Integrated Care Subgroup is leading the work to understand our current performance, and to develop plans to turn it around. We are inviting you to a workshop that will help us develop a shared understanding and collaboration around the hospital admissions in Barking and Dagenham. We want to hear from a wide a range of views from as many organisations as possible.
The workshop objectives are:
• To engage a wider cross-section of our partnership in an understanding of what is driving hospital admissions.
• To test some hypotheses that we have developed based on analysis of the data, benefiting from stakeholders’ experiences of hospital admissions.
• To agree actions that will improve our performance, and which can contribute to a reshaping of our Better Care Fund plan.
The workshop outcomes will be documented and reported to the HWBB Integrated Care Subgroup and Joint Executive Management Committee.
If your organisation represents people using services, carers, or a community, the person attending should have a good understanding of the topic from a patient, carer or community point of view.
